Severe
impaired 46 year male consultant, microdiscectomy left L4-5 24 February
2000 and microdiscectomy left L5-S1 14.09.2001 , backpain > legpain, back
3/10 leg right 10/10, leg left 5/10, some neck and interscapular pain,
walking 10 minutes, sitting 30 minutes, irradiation heel left, poor sleeping,
mild(10%) restrictive(85%) unbearable (5%), increasing, brothers have
spinal problems, numbness left leg, weakness left leg, ADL 30%, aspects
of life fairly disappointing, Roland 19/24, Oswestry 62% (= severe impairments)
X rays
X lumbar spine: L4-5 and L5-S1 diminished height, no transitional vertebrae,
MRI lumbar spine: black disk L4-5 and 5-S1, sclerotic endplates L4-5 Discography
painfull at L45 and L5S1 ? Discography: L45 > L5S1 concordant pain.
Diagnosis
1 severe impaired by chronic back-leg pain syndrome left 2 postdiscectomy
syndrome L4-5, L5-S1 3 symptomatic DDDD L4-5 >>> L5-S1
